The facility failed to intervene and timely seek medical treatment when the Alleged Victim (AV) experienced a fall and sustained a bump on the head and injury to his/her arm, placing AV at a risk for serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
