Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. The AV had two falls and suffered skin tears each time, causing undue pain and suffering. On or about November 19, 2019, Alleged Perpetrator #2 (AP2) failed to properly assess AV after his/her fall for injuries. The next day, staff found AV to have two skin tears to his/her left leg. Alleged Perpetrator #3 failed to timely answer a call light for AV, resulting in AV falling and receiving two skin tears. The facility, AP2 and AP3’s failures are a violation of resident rights, are considered neglect of care and constitute abuse.
