Findings
On or about November 22, 2019, Witness 1 (W1) lifted up Alleged Victim's (AV) shirt without AV's consent, causing AV emotional harm. W1 had known inappropriate behaviors and the facility failed to care plan according to those behaviors. An investigation determined that W1 demonstrated inappropriate sexual behaviors to staff since July 17, 2019. The facility failed to put interventions in place for W1 to protect others from W1's inappropriate behavior which is a violation of resident rights, is considered neglect of care and resulted in negative behavior.
