Findings
On or about November 25, 2019, Alleged Perpetrator 2 (AP2) slapped Alleged Victim (AV) on the face while assisting AV with his/her Activities of Daily Living which caused AV's cheek to become red and swollen. AP2's actions are considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
