Findings
The facility failed to assure the Alleged Victim (AV) was safe according to his/her needs and fall history. The failure resulted in AV experiencing an unwitnessed fall causing a broken collarbone, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
