Findings
Alleged Victim (AV) was care planned to wear arm sleeve protectors daily. On or about November 23, 2019, Alleged Perpetrator #2 (AP2) and Alleged Perpetrator 3 (AP3) failed to follow the Alleged Victim's (AV) care plan by putting a long sleeve shirt on AV instead of a sleeve protector resulting in a skin tear to AV’s arm. The facility AP2's and AP3’s actions are considered neglect of care and constitute abuse. The facility failed to ensure AV’s care plan was followed, which is a violation of Oregon Administrative Rules.
