Findings
On or about November 30, 2020, at approximately 12:00pm, Alleged Victim (AV) was cleaning the dining room table with a washcloth and Witness 1 (W1) refused to move, so AV put the washcloth on W1's neck. AV and W1 started yelling at each other. Alleged Perpetrator 2 (AP2) saw the verbal altercation between AV and W1 but walked away without intervening. W1 reached out and grabbed AV's arm and hit AV. AP2's failure to intervene is considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect which is a violation of Oregon Administrative Rules.
