Findings
Alleged Victim (AV) resides in a secured memory care unit due to his/her needs. On or about December 3, 2019, AV left the facility unattended putting AV at risk for serious harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
