Findings
The facility failed to provide proper supervision to maintain the health and safety of Alleged Victim (AV), resulting in unreasonable discomfort to AV. An investigation determined that the facility failed to follow AV's care plan by not providing appropriate peri care according to his/her care plan. As a result, AV developed a pressure sore on his/her buttock area. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
