Findings
The Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 failed to perform proper safety checks and assure the Alleged Victim was safe. The failure resulted in AV being found on the floor approximately 9 hours after last being checked and received multiple skin injuries and hospitalization, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from harm which is a violation of Oregon Administrative Rules.
