Findings
The facility failed to follow the Alleged Victim's (AV) care plan to to provide a two-person transfer. During an investigation, it was discovered that no staff knew that AV was a two-person transfer, and staff were transferring AV with one staff only. This practice places AV at a risk of serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
