- Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan to have hands on while showering. During the shower, the AP2 left AV standing in the shower alone to grab supplies and the AV fell. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failed to provide a safe environment and conduct an internal investigation of a prior incident where AP2 also left a resident in the shower and a fall occurred, placing residents at a risk for serious harm.
