Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 and the Alleged Victim. An incident occurred between the two where Witness #1 grabbed the Alleged Victim around the neck, causing redness to his/her shoulders. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
