Findings
Alleged Victim (AV) and Witness 1 (W1) have a history of aggressive behavior when an unwanted person gets too close to them. On or about December 12, 2019, W1 hit AV with his/her cane when AV came to close to W1. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
