Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to witness #1. On or about December 10, 2019, an incident occurred between the Alleged Victim and witness #1 where witness #1 hit the Alleged Victim in the face, causing a nose bleed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
