Findings
The Alleged Victim (AV) must be transferred with the assistance of facility staff. The facility failed to answer the call light in a timely manner for the Alleged Victim while he/she was left in the restroom for more than 20 minutes after pressing the call light, causing unreasonable discomfort and which places AV at a risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
