Findings
The facility failed to follow the Alleged Victim's care plan to remind him/her to use his/her walker or a wheel chair during ambulation, or to observe him/her while in the common area to determine if he/she is showing weakness and remind him/her to use the walker. The Alleged Victim did not suffer any injuries, however, the facility placed him/her at a risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
