The facility failed to follow witness #1’s care plan to be supervised and have 1:1 care. Witness #1 pulled on the Alleged Victim's arm, causing him/her to fall to the ground. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
