Findings
The facility failed to provide appropriate oversight and monitoring regarding the Alleged Victim's (AV) change of condition. The failure resulted in AV experiencing multiple falls and experiencing repeated unreasonable discomfort before finally receiving a medical evaluation where it was determined he/she had experienced a fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
