- The facility failed to follow the Alleged Victim’s (AV) care plan to complete safety checks 32 times per shift, during which time the AV fell and was transferred to the hospital where it was determined he/she suffered a broken hip. On or about December 22, 2019, Alleged Perpetrator #2 was advised that AV was ready to get up for the day at approximately 6:00 am, however, at approximately 6:15 am, AV was found on the floor from a fall. AV was care planned to be helped from bed at 6:00 am every day. AP2 failed to check on the AV as soon as he/she was advised AV was awake and ready to get up for the day. AP2’s and the facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
