- On or about December 22, 2019, Alleged Perpetrator #2 (AP2) was applying the Alleged Victim's TED hose while wearing a ring with a large, protruding stone. The stone of AP2's ring snagged on AV's right leg, causing a deep skin tear, which required stitches. AP2 failed to follow facility policy to not wear jewelry that could cause injury to residents. AP2's actions are considered neglect of care, which constitutes abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
