Findings
The facility failed to follow Witness #1’s (W1) care plan to redirect when W1 is near the Alleged Victim (AV). On or about December 22, 2019, W1 hit AV in the back of the neck, causing a risk of harm to AV. The facility's failure to follow the care plan and provide a safe environment for residents is a violation of resident rights, is considered neglect of care and constitutes abuse.
