Findings
On or about December 27, 2019, Alleged Victim (AV) was sent to the hospital and was diagnosed with pressure wounds on both of his/her heels and also on his/her private area. The hospital determined that the pressure wounds were due to AV remaining unmoved in his/her bed for long periods of time. An investigation determined that AV was not able to reposition self due to AV's hanging grab bar being removed by the facility. The facility's failure to provide service to AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
