Findings
The facility failed to follow witness #1’s care plan to keep him/her away from other residents to prevent altercations. On or about January 2, 2020, witness #1 hit the Alleged Victim in the face, causing him/her to be sent to the hospital for evaluation. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
