Findings
The facility failed to provide appropriate supervision to the Alleged Victim (AV) and ensure his/her safety. The failure resulted in AV being left unattended for approximately five hours with injury and unreasonable discomfort in low temperatures, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
