- Alleged Victim (AV) is listed as a fall risk but independent with mobility, transfers, toileting, dressing and grooming. On or about December 16, 2019, AV self-reported to staff having three falls recently. A progress note dated January 1, 2020, states AV was assessed and evaluated for the reported falls on or about December 19, 2019, which concluded AV had full range of motion and no apparent injury. AV was put on alert charting on or about December 19, 2019. On or about December 30, 2019, AV was taken to his/her PCP by family due to AV complaining of pain and not going down to meals for the prior two days. The facility failed to timely assess AV after he/she reported the falls and the facility failed to respond to AV's change of condition two days prior to going to the hospital, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
