Findings
On or about December 21, 2019, Alleged Perpetrator 2 (AP2) and another staff were assisting Alleged Victim (AV) with dressing when AP2 yanked up AV’s right arm, causing AV to scream in pain. AP2’s action is a violation of resident rights and is considered physical abuse. The facility failed to protect AV from physical abuse which is a violation of Oregon Administrative Rules.
