Findings
On or about January 9, 2020, Alleged Victim (AV) had an unwitnessed fall that resulted in pain to the back of AV's head. An investigation determined that the facility failed to follow AV's care plan which a violation of resident rights, is considered neglect of care and constitutes abuse.
