Findings
The facility and Alleged Perpetrator 2 failed to protect the Alleged Victim from skin injury while transferring him/her or assisting with ambulation. The failure resulted in AV experiencing bruising and injury to his/her hand, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
