Findings
Facility staff manages Alleged Victims (AV's) medications. AV suffers from chronic pain and takes a pain patch to be applied every three days. On or about January 6, 2020, AV's pain patch was not applied causing AV unreasonable discomfort. Through the investigative process it was also discovered the facility is missing at least one of AV's pain patches. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is neglect of care and constitutes abuse.
