Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to witness #1. On or about January 14, 2020, an incident occurred between the Alleged Victim and witness #1 where witness #1 grabbed and drug the Alleged Victim down the hall, causing bruising to his/her hand. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
