Findings
The facility failed to follow witness #1’s care plan to provide 1:1 care to prevent altercations. On or about January 14, 2020, witness #1 grabbed the Alleged Victim's wrist, causing the Alleged Victim to hit witness #1 in the chest. There were no injuries, however, the incident placed both residents at a risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
