Findings
Alleged Perpetrator 2 (AP2) was assisting the Alleged Victim with personal care on 1/12/20 and on 1/13/20, AV had a bruise on his/her left hand. AP2 was rough with the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse. AP2 was found not substantiated for physical abuse in this case. The facility failed to provide a safe environment, which is a violation or Oregon Administrative Rules.
