Findings
The facility failed to ensure the Alleged Victim (AV) was safe when he/she fell and it took 43 minutes for care staff to answer AV's call light. AV sustained swelling and discoloration to his/her ear and was sent to the hospital for evaluation. The facility's failure is a violation of Oregon Administrative Rules.
