Findings
On or about January 16, 2020, Alleged Victim (AV) left the facility without assistance while AV was left unsupervised on the facility's back patio. AV is care planned to be supervised at all times while out on the back patio. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care, and constitutes abuse.
