- Alleged Victim (AV) reported his/her phone had additional charges associated with international calls from AV's phone line. AV also reported his/her property went missing, some of which were gifted to staff. The facility's policy states that staff are not to accept gifts from residents. The facility failed in the assumed responsibility to the AV, resulting in unknown phone charges on AV's phone bills, missing property, facility staff accepting gifts from AV in violation of facility policy and money missing from AV's room. An unknown person is responsible for the thefts which is considered financial exploitation and constitutes abuse. The facility failed to protect AV from repeated financial exploitation which is considered neglect of care and constitutes abuse.
