The facility failed to ensure supervision and staff support regarding known behaviors related to the Alleged Victim, Witness #2 and Witness #3. An incident occurred between the three where Witness #2 and Witness #3 were seen hitting the Alleged Victim. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
