Findings
The facility failed to provide a safe environment and proper supervision for the Alleged Victim. Between 12/8/2019 - 1/23/2020 the AV experienced approximately five altercations with other residents and four noted injuries, one of which required AV to be sent to the hospital for sutures. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
