Findings
Alleged Victim (AV) is care planned as an elopement risk and had a history of elopement. On or about January 18, 2020, AV successfully exited the facility and unlocked and opened the back gate. AV was found outside of the gate, uninjured. The facility failed to provide a safe environment placing AV at risk for harm which is a violation of resident rights, is considered neglect of care and constitutes abuse.
