Findings
Alleged Perpetrator 2 failed to provide safe medication administration to ensure the Alleged Victim (AV) received his/her medication on or about January 17, 2020. The failure placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure a safe medication administration system which is a violation of Oregon Administrative Rules.
