Findings
On or about January 2, 2020 Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (A)3) failed to follow the care plan for the Alleged Victim (AV) by not removing the footrest on the wheelchair prior to transferring him/her, causing injury to AV's leg. AP2 and AP3's actions are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
