- The facility failed to implement appropriate interventions and staff support regarding known fall risks related to the Alleged Victim (AV). AV has had multiple falls resulting in large bruises, however, no new interventions were put in place to staff to know how to reduce the risk of falls for this resident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
