Findings
On or about January 21, 2020, Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim's (AV) care plan which resulted in AP2 causing AV pain and discomfort while AP2 assisted AV with dressing. AP2's actions are considered physical abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
