Findings
The facility failed to provide a safe environment for the Alleged Victim (AV) by not placing his/her call pendant where he/she could easily reach it. AV tried to transfer by him/herself and had an unwitnessed fall without injury, which placed AV at a risk of harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
