Findings
The facility failed to follow the Alleged Victim's (AV) care plan to place Geri sleeves on him/her to prevent skin tears and bruising. On or about January 9, 2020, AV was found to have skin tears and bruising to both forearms, and AV was not wearing Geri sleeves as care planned. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
