Findings
The facility failed to appropriately care plan related to the Alleged Victim's (AV) fall history. The facility had inappropriate interventions of reminding AV to use his/her call light when AV has limited memory capacity and/or ability to understand his/her care needs. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
