Findings
Alleged Victim (AV) resides in a memory care and is diagnosed with cognitive impairment. On or about February 4, 2020, AV was found to have an orange substance all over his/her face and hands. The orange substance was determined to be paint that was located on a shelf that was not secured against resident's access. AV was monitored and experienced diarrhea as a result. The facility failed to provide a safe environment causing AV unreasonable discomfort and putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
