Findings
On or about February 5, 2020, Alleged Victim (AV) used his/her call light for staff assistance. Alleged Perpetrator 2 (AP2) responded and called AV a derogatory name and told AV to not use his/her light again. AP2's actions are considered verbal/emotional abuse. The facility failed to protect AV from verbal/emotional abuse, which is a violation of Oregon Administrative Rules.
