Findings
The facility failed to follow the Alleged Victim's (AV) care plan to have an active bed alarm, or to respond to the bed alarm when activated by the AV. On or about February 4th or 5th, 2020, the bed alarm in AV's room either didn't get responded to or was turned off, AV fell and was transported to the hospital and diagnosed with 4 fractured ribs. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
