Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to witness #1. On or about February 7, 2020, an incident occurred between the Alleged Victim and witness #1 where witness #1 pushed the Alleged Victim, causing him/her to fall. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
